This is for my green card app through family-based category. Which medical forms or vaccinations are needed? Is there an expiry on the signed form? thanks.

A:You will have to prove that you have had the following vaccinations: Measles, Mumps, Rubella, Chicken Pox, Polio, You will have to test negative for the following: TB, HIV, Syphilis They will also ask you if you suffer any of the following: Alcholism, Gonorrhea, Mental Defects, Drug Addication, or Insanity (I swear this is all on the form!) I believe that the immigration physial is good for 6 months (not sure on that though) you should have it the week before you intend to file as it takes 3-5 days to get some of the test reults back.

i dont really have much to spend and i practice medical assisting so where can i find free froms to print out and use?

A:There are an infinite number of 'forms' available - what type of forms do you require? A good place to start is with a text book of the field of medicine in question. At this time - much to my amazement - virtually every book is available online without charge if you spend a few minutes looking. There are books of forms - books of flow charts - and books of algorithms. If I know what you are seeking I will be of more precise assistance.

100% Q: Are Chiropractors able to fill out forms for say a week of medical leave due to a car accident/back injury?

I took a few days off after a rear end collision and after 3 days my work wants a "treating medical professional" to fill out some forms for a medical leave of absence. My family physician says I can go back after one week. My Chiropractor says I should not perform physical work for 3 weeks or until my pinched nerve/muscle spasms start to go away. Can a Chiropractor make these decisions and fill out a medical leave form?? I'm not lazy.....I just dont want to aggravate an ongoing back issue!

A:I agree with the answer at the top, but you must fill out an accident questionaire while you are at the office and also return for appointments should it ever be brought up in an arbitration or in court. If you miss any appointments it may be misconstrued. If you are using your PIP for payment please be sure to follow through with appointments and get the Chiro. treatment you need. A Chiropractor CAN give you a "leave of absence" but they hesitate to give you long-term because of their position in the medical community and the lack of respect brought on by long term "backward thinking". I signed medical release forms for a lawyer regarding a car accident. Will they be able to get my psychology notes that I consider very private and have nothing to do with my injuries. They are held at the same clinic and are about 10 years old!

A:No, your lawyer will not be able to obtain your psychological records with the HIPPA release forms you signed for you PI case. The forms you signed are so they can obtain your hospital, chiropractic, physical therapy, etc. records. On the forms you signed, you have to initial your permission for psychological records. If you didn't initial that portion, your lawyer cannot obtain that information. And they wouldn't ask to obtain those records unless it was part of your litigation.

100% Q: Skilled worker immigration to Canada: Adding My child After for Getting the Medical Forms???

My application for Skilled Worker immigration was in process when I was blessed with a baby girl last April. I applied for a US passport for the baby as soon as I got her birth certificate. The whole process (birth certification and passport processing) took about 4 months, and I just got her passport last week. Yesterday, in the mail I received the medical report forms (section A) for both: me and my wife. What should I do now? Do I go ahead with completing the medical forms and health tests needed? Or I apply for my daughterâ€™s addition to my application for permanent residency first? And what are the steps for each? Thanks in anticipation.

A:Apply for your daughter first. The medical is only good for 1 year so it must be timed within that.
